Referrals
Accessing our exercise exercise physiology and physiotherapy services requires a referral from a healthcare provider, such as:
- Local Medical Officers (LMO)
- General Practitioners (GP)
- Medical specialists
- Another exercise physiologist transferring the entitled person
- Hospital discharge planners
- Treating doctors in hospitals
Referrals should be documented on a DVA Request/Referral Form (Form D904) or official practitioner's letterhead, including essential details such as:
- Name of the entitled person receiving services
- DVA file number of the entitled person
- Referrer's healthcare provider number (or hospital's provider number if discharge referral)
- Date of referral
- Condition to be treated
